How Our Weight Management Pathway Works

 

Step 1 – Online consultation form
Patients complete a detailed online consultation form, including medical history, current medications, and lifestyle information. This is reviewed by a registered Nurse Practitioner. Patients will receive a response within 48 hours.

Step 2 – Identity & eligibility checks
Before treatment is approved, patients are required to submit:

  • Photo ID (e.g. driving licence, or passport and utility bill with address on ).

  • A current full-body photograph; (with face showing, and not in baggy clothes).

  • A photograph of their weight clearly displayed on scales display.

This helps ensure safe, appropriate prescribing and ongoing monitoring.

Step 3 – Clinical review & approval
All information is reviewed to determine whether treatment is clinically appropriate.
Approval is based on safety, eligibility, and individual health factors.

Step 4 – Access to the patient portal

Once a patient has been assessed and confirmed as clinically suitable, they are invited to create an online account.

Patients may view general information, however they cannot place any orders or submit treatment requests unless their account has been manually reviewed and approved by the clinic.

Only after account approval can a patient proceed to request treatment as part of their agreed care plan.

Please note:

  • All treatment requests are subject to a further clinical check prior to prescribing.

  • The dose and timing of any prescription are reviewed again before issue.

  • If a request is not considered clinically appropriate, the patient will be contacted to explain the decision and a refund will be provided where applicable.

 

Creating an account or obtaining account approval does not guarantee that medication will be prescribed.

Step 5 – Prescription & pharmacy supply
If medication is clinically appropriate, a prescription is issued and sent to our UK-registered partner pharmacy, who dispense and deliver the medication directly to the patient. 

Prescriptions are dispensed by Somer Pharmacy, a UK-registered pharmacy regulated by the General Pharmaceutical Council (GPhC registration number: 9011581) Unit 10 Willowbrook Technology Park, Crickhowell Road, St Mellons, Cardiff CF3 0EF

Step 6 – First injection support (video consultation)
Once the first pen has arrived, patients attend a video consultation where:

  • The first injection is administered together

  • Injection technique is demonstrated

  • Side effects, expectations, and dose escalation are discussed

  • Diet and lifestyle changes discussed

Step 7 – Ongoing guidance & automated support
Patients receive text guidance at key points (e.g. day 2, 5, one week and week 3) with instructions on injection technique, dose changes, and what to expect, and a contact number for immediate support.

Step 8 – Monitoring & follow-up
Patients complete a review form after 4 weeks to monitor:

  • Weight changes

  • Side effects

  • Tolerance of treatment

  • To check they remain clinically suitable to obtain their repeat prescription for the next month.

Ongoing support is available 24/7 for any concerns or questions.

Step 9 – Repeat prescriptions & reviews

Repeat prescriptions are issued only where clinically appropriate and following reassessment by a Nurse Practitioner.

Patients are required to:

  • Submit regular progress updates, including monthly (4-weekly) review forms to assess weight loss, side effects, and overall suitability for continued treatment. Suitability is reassessed every four weeks, and sooner if required. If a patient is deemed unsuitable at any stage, no further prescriptions will be issued.

  • Attend a scheduled video consultation every three months, during which:

    • Updated full-body photographs are reviewed

    • Weight verification is checked

    • Progress, safety, and ongoing suitability are assessed

    • The patient is visually assessed as part of their clinical review

This process ensures treatment remains safe, appropriate, and tailored to the individual patient.
